To help expedite your first visit, please fill out the Registration Form prior to your appointment.
You can also help us expedite the assessment process by having the following information gathered (where applicable) at
- A panoramic X-ray taken within the past six months. If you do not have one available we will be happy to take one at your initial appointment.
- Your dental insurance card. By providing this information at the first visit, we will be able to give you a more accurate estimate of your costs.
