  • Recent Panoramic X-ray: If you have a panoramic X-ray taken within the last six months, please bring it with you. If not, we’ll be happy to take one during your initial appointment to ensure we have a complete view of your oral health.
  • Dental Insurance Card: Bringing your insurance information allows us to provide you with a more accurate estimate of any out-of-pocket costs for your treatment.
